Going beyond prepared meals in Transition of Care Programs

A recent study found that members receiving food (prepared meals) post-discharge from the hospital experienced fewer readmissions than those not signing up for the program.

The study also identified, however, that there were several reasons eligible members did not sign up for the program, explicitly pointing to members stating they cook themselves or have family and friends do so.

“In some cases, the member could not be contacted, and in others they declined to use the meal benefit or said they already had an option for getting meals, either by cooking it themselves or having family and friends do so.”
